    IDOI’s FAQ states that physicians and independent ancillary providers working through emergency-room groups or physician-owned facilities file individual proof and pay the appropriate surcharge; an ancillary provider cannot include those individual providers in its qualification. Have the facility and clinicians verify their own qualification and coverage instead of relying on the institution’s excess-limit certificate. 1
