A consent order dated 2025-12-18 (No. 2025-9693) imposed $59,825 for late filings in 2024. A consent order dated 2024-10-03 (No. 2024-8885) imposed $28,300 for late filings in 2023. The 2025 order lists earlier orders from 2017, 2018, 2020 and 2024. These are paperwork-timing violations, not findings that claims were mishandled.
