Arizona continuation covers qualifying events for eligible people when the employer averaged at least one but fewer than 20 eligible employees in the preceding calendar year and federal COBRA is unavailable. Enrollees need three months of prior plan coverage. Arizona uses an average of at least one but fewer than 20 eligible employees during the preceding year, not a point-in-time count. Ask the carrier whether federal COBRA or the state rule governs before estimating continuation costs.[^1]

### Track Prior Coverage And Full Cost

The enrollee needs three months of plan coverage before the event and pays full cost plus an employer administrative fee capped at 5% of premium. Confirm effective dates and cost before communicating the option. Ask the issuer to confirm the contract is a qualifying group health benefits plan; the chapter definition excludes products such as accident-only, specified-disease, dental-only and vision-only coverage.[^1][^2]

### Build Notice Deadlines Into Offboarding

The employer generally must notify the enrollee within 30 days; a notice postmarked within 44 days after the event and mailed to the last known address satisfies that requirement. With complete, accurate and timely notice, the enrollee has 60 days after the notice to elect and 45 days after election to pay the first month; if notice is incomplete, inaccurate or late, the enrollee has 120 days after the notice to elect and pay. A separate notice is required for a known dependent who lives elsewhere when the employer knows that dependent’s address.[^1]
