# Fill in an ACORD 25 certificate from your policies

Generate an ACORD 25 certificate of liability insurance in the Spot app from your own policies, and know what each section of the form shows.

- Each section filled in from your actual policies
- Endorsements checked before the form marks them
- Keep your broker or switch, with no commissions on premiums

## How it works

- **Keep your policies in Spot.** Your declarations, forms and endorsements live in your Spot workspace, so the certificate draws from the actual policies.
- **Check the request first.** If the request asks for an endorsement your policy doesn’t have, your Spot team requests it through your broker and insurer before the form says so.
- **Generate the ACORD 25.** Create the certificate in the Spot app for the holder who asked, and send it.

## What your certificate needs to show

- **Producer and insured.** The top of the form shows the broker who placed the coverage and your business’s name and address, and the date the certificate was issued.
- **Insurers affording coverage.** Each insurer on the certificate is listed by letter with its NAIC number, and every policy row points to one of those letters.
- **Coverages.** A row for each policy: general liability, auto, umbrella or excess, workers’ compensation and others, with policy numbers, effective and expiration dates and limits.
- **Additional insured and waiver boxes.** Each row has a box for additional insured and one for subrogation waived. They should only be marked when the policy has the endorsement.
- **Description of operations.** Free text for the project, location, vehicles or contract, and for references to endorsements the policy carries.
- **Certificate holder and cancellation.** The party who asked for the certificate, a note that cancellation notice follows the policy provisions, and an authorized representative’s signature.

## Endorsements to check

- **What the form itself says.** The ACORD 25 states that it’s issued for information only, gives the holder no rights, and doesn’t amend, extend or alter the coverage of the policies listed.
- **Additional insured and waiver of subrogation.** The form notes that an additional insured needs the policy to have that provision or be endorsed, and that a waiver may also need an endorsement. Your Spot team requests these through your broker and insurer.

## Generate your certificate in Spot

- **Choose the liability policies.** Pick the policies the request asks to see. Each becomes a row in the coverages section.
- **Enter the certificate holder.** Add the requester’s name and address for the holder box.
- **Upload the request.** Attach the request or list the requirements and any text for the description of operations.
- **Review the finished form.** Read every row, box and the description against the request, then download the ACORD 25 or send it.

## Mistakes to avoid

- **Using it for property evidence.** The ACORD 25 is a certificate of liability insurance. Lenders and landlords asking about property may want a property form such as ACORD 28.
- **Overloading the description box.** Wording in the description that the policy doesn’t support changes nothing, and reviewers may reject it.
- **Out-of-date policy rows.** Policy numbers and dates change at renewal. Generate a new certificate rather than resending an old one.

## Common questions

### What is an ACORD 25?

The standard certificate of liability insurance form used in the United States. It summarizes your liability policies for someone who asked for proof.

### Can I fill in an ACORD 25 myself?

Spot customers generate it in the Spot app from their own policies, so the rows match what was bound.

### Does the ACORD 25 add coverage?

No. The form says so itself. Any additional insured, waiver or primary and non-contributory terms have to be on the policy, and your Spot team requests them through your broker and insurer.

### Does my broker still need to be involved?

Your broker placed the policies and stays on them. The certificate comes from those policies in your Spot workspace, and Spot takes no commissions on premiums.
